Before coming to our center for pre-specified email address inmate or family member is sent a list of the documents, please complete and sign. These are: 

• model contract 
• referral to the resort issued and signed by the doctor treating the patient so far 
• rules of the center 

The documents needed and useful during the reception include: 

• identity card or other document confirming the identity of the inmate 
• all documents relating to the history of disease, eg hospital sheets, information from the GP, medications, information about allergies and vaccinations. All we need medical information are included in the document referrals, the model of which is sent electronically
• confirmation of insurance in the NFZ proven system eWUŚ. In the absence of confirmation eWUŚ insurance system will not be able unsubscribing reimbursed prescriptions and the provision of free services GP and specialists until the correct documents in the system eWUŚ 
• declaration of accession to the primary care physician 

Residents positively verified in the system eWUŚ on admission for a longer period have the opportunity to sign the declaration of accession to the Clinic Medical Centre Manamedica Ltd. , which is located in the immediate vicinity of the resort. This solution increases the availability of inmate to free medical care in the context of: 

• primary health care 
• care professionals 
• allows writing out prescriptions reimbursed and such requests for diapers or wheelchairs 

In case of failure to submit a declaration to the / clinic or a lack of insurance or family boarder boarder himself all of the above steps takes care of its own and at their own expense
